Evidence-based investigation
Learners practise separating facts from assumptions and examining the difference between work as planned and work as performed. The programme considers human, technical and organisational factors rather than stopping at “unsafe act” or blaming the injured employee.
Root causes and corrective actions
Depending on the selected course scope, activities may use five-whys, barrier analysis or structured causal questioning. Corrective actions are evaluated against the hierarchy of controls and assigned owners, deadlines and verification measures.
South African reporting context
Section 24 of the Occupational Health and Safety Act and the applicable General Administrative Regulations contain important incident-reporting and investigation requirements. Training provides general operational understanding; organisations must obtain competent advice for a specific serious or reportable event.
